What Does Medicare NOT Cover? The Gaps That Surprise People

Most folks assume Medicare handles everything once they turn 65. Then the first dental bill or hearing-aid quote arrives. Here's what Original Medicare leaves out — and what people actually do about it.

Dental care

Original Medicare does not cover routine cleanings, fillings, crowns, dentures, or implants. Some Medicare Advantage plans include dental benefits, and standalone dental plans are widely available. Given what a single crown costs, this gap matters.

Vision care

Routine eye exams, glasses, and contacts aren't covered by Original Medicare (one exception: an annual eye exam if you have diabetes). Many people pair their coverage with a vision plan or choose an Advantage plan that bundles it.

Hearing aids

Hearing exams and hearing aids are not covered by Original Medicare — and hearing aids run into the thousands. Some Advantage plans offer a hearing benefit with an annual allowance.

Long-term care

This is the big one people miss. Medicare covers short-term skilled nursing after a hospital stay — not custodial long-term care, assisted living, or nursing-home stays. Planning for this is a separate conversation, and it's worth having before you need it.

Prescription drugs (without Part D)

Original Medicare Parts A and B don't include outpatient prescriptions. You need a Part D drug plan or an Advantage plan with drug coverage — and enrolling late can trigger a permanent penalty.

Care outside the U.S.

Planning to travel? Original Medicare generally doesn't cover care abroad. Some Supplement plans and travel policies do.

There's also no cap on your costs

Original Medicare has no annual out-of-pocket maximum. A serious illness can mean 20% of very large bills, with no ceiling. That's why most people add either a Medicare Advantage plan (which by law caps your yearly costs) or a Medicare Supplement.

The bottom line

Medicare is a strong foundation — it just isn't a finished house. The smart move is a free annual review to make sure your coverage actually covers your life: your doctors, your prescriptions, your teeth, your eyes.
